About the job:

Pay: $19.40 - $24.50 (based on experience)

Schedule: M-F, 4 Days per week - 10 hour shifts , Occasional weekends and evenings required.

Counties that we also travel to:
Grant, Gallatin, Carrollton county areas.

The Home Care CNA
is responsible for providing and assisting participants with activities of daily living and restorative/supportive care necessary to ensure the physical and health safety of the home environment. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior Community Care of State (SCC). As assigned, works shifts in the SCC Day Center and in participant homes and provides care and services to participants and assists with the provision of a safe and effective day program.

Required Qualifications:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

• Must have an active and up-to-date certification in CPR.
• Must have - Current STNA license
• A minimum of one-year experience in working with the frail or elderly population.
• High School diploma or equivalent required.
• Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
• Must have a valid driver’s license, proof of insurance and have means of transportation. You will be traveling to participant homes.

Preferred:

• Bilingual skills are a plus!
• Consults each day with the Home Care Coordinator (or designee) to be updated on changes in participant status, changes in plan of care, and to confirm scheduled visits.
• Assists participants in the home with the following duties:
  • Personal hygiene such as skin, mouth, hair care and bathing.
