St. Tammany Health System in Covington, LA is seeking an RN Care Coordinator for a part-time role (20 hours per week), with 8-hour day shifts and a weekend shift every six weeks.
The goal is to coordinate care, assess and plan to meet patient needs while ensuring quality and cost-conscious delivery. Minimum qualifications include graduation from an accredited nursing program, Louisiana RN license, 3–5 years in hospital adult acute care, BLS certification, and CCM/CMAC/ACM preferred.
