Easterseals Louisiana seeks a Social Services Coordinator to support families in completing the initial IFSP with SPOE. You’ll coordinate services across agencies and be the primary contact to help families access needed supports and providers identified on the IFSP.
You will assemble multidisciplinary teams for annual eligibility reviews, ensure timely services, assist with transitions from Early Steps at age 3, and maintain required licenses and trainings.
