- Schedule:
Monday to Friday
- Assess and evaluate patient’s status.
- Write and initiate plan of care.
- Regularly re‑evaluate patient and family/caregiver needs.
- Participate in revising the plan of care as necessary.
- Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, including the patient and the family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician in the physician’s plan of care.
- Submit accurate and relevant clinical notes regarding patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate.
- Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physicians’ orders as required.
- Provide and maintain a safe environment for the patient.
- Attend interdisciplinary group meetings.
- Participate in on‑call duties.
- Ensure that arrangements for equipment and other necessary items and services are available.
- Supervise ancillary personnel and delegate responsibilities when required.
- Assume responsibility for personal growth and development and maintain and upgrade professional knowledge and practice skills through attendance and participation in continuing education and in‑service classes.
- Fulfill the obligation of requested and/or accepted case assignments.
- Actively participate in quality assessment performance improvement teams and activities.
- 2+ years’ nursing experience, preferably in geriatrics.
- Degree in Nursing from an accredited institution.
- Proficient with electronic medical records.
- Excellent bedside manner and communication skills.
- Working knowledge of health‑care laws and regulations.
- Maintain current BLS CPR certification.
- Possess a valid driver’s license and own a reliable, insured vehicle.
- Salary Range: $72K‑90K Annual Salary
