Med Care Manager

Sunrise Senior Living seeks a Med Care Manager to coordinate and deliver high-quality care for residents in our senior living community. In this resident-centered role, you will manage safe medication administration, perform ongoing health assessments, and update individualized care plans. You'll collaborate closely with physicians, pharmacies, and a compassionate care team, ensure regulatory and infection control compliance, and maintain accurate documentation.

You will also educate and support care staff, and build trusting, respectful relationships with residents and families every day.

Responsibilities

• Oversee and perform safe medication administration and documentation for residents.
• Assess residents' health status and update individualized care plans.
• Coordinate with physicians, pharmacies, and care teams to support resident needs.
• Monitor changes in condition and initiate appropriate interventions.
• Ensure compliance with state regulations, policies, and infection control standards.
• Lead, coach, and support care staff in delivering high-quality resident care.
• Maintain accurate electronic health records and care documentation.
• Communicate regularly with residents and families regarding care and changes in condition.
• Participate in care conferences, quality initiatives, and training programs.
• Support onboarding and ongoing education of care team members.
