We are currently seeking a compassionate and motivated Occupational Therapist (OT) to provide home health services throughout Iowa. This rewarding opportunity allows you to work directly with patients in their homes, helping them improve independence, safety, and overall quality of life through individualized occupational therapy services.
Position OverviewThe Occupational Therapist will evaluate and treat patients experiencing physical, cognitive, developmental, or psychosocial limitations that affect their ability to perform activities of daily living. The OT will develop and implement personalized treatment plans designed to maximize functional independence while collaborating with patients, caregivers, physicians, and interdisciplinary healthcare teams.
Responsibilities- Conduct comprehensive occupational therapy evaluations in patients' homes.
- Develop individualized treatment plans based on patient needs, goals, and physician referrals.
- Provide therapeutic interventions to improve activities of daily living (ADLs), instrumental activities of daily living (IADLs), mobility, coordination, strength, and cognitive functioning.
- Assess home environments and recommend modifications or adaptive equipment to improve safety and independence.
- Educate patients and caregivers on treatment strategies, adaptive techniques, and home exercise programs.
- Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and other healthcare professionals to coordinate care.
- Monitor patient progress and adjust treatment plans as appropriate.
- Complete accurate and timely documentation in compliance with Medicare, state, federal, and agency regulations.
- Participate in care conferences and interdisciplinary team meetings as needed.
- Graduate of an accredited Occupational Therapy program with a Bachelor's, Master's, or Doctoral degree in Occupational Therapy.
- Current Iowa Occupational Therapist License issued by the Iowa Board of Physical and Occupational Therapy.
- Current CPR/BLS certification or ability to obtain prior to start.
- Valid driver's license, reliable transportation, and current auto insurance.
- Ability to travel throughout assigned service areas.
- Strong communication, clinical reasoning, and documentation skills.
- Home health experience preferred but not required.
