The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans to meet patient needs, goals, and physician directions. The therapist delegates and directs all discipline‑specific therapy services for assigned patients, ensuring functional treatment programs that facilitate return to community activities. They serve as an educator to patients, families, students, and colleagues, and perform other professional duties associated with patient care and therapy service development.

• Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans focused on discharge readiness.
• Maintain clear, concise, accurate, and timely medical record documentation, ensuring compliance with state, federal, and clinic policies.
• Participate in student training programs by providing a positive learning environment and conducting student evaluations.
• Create and actively pursue an individualized professional development plan in collaboration with clinical management.
• Adhere to discipline‑specific State Practice Act and maintain professional licensure in good standing, completing all continuing education requirements.
• Engage in organizational performance improvement initiatives to enhance the quality of services provided.

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active state licensure for Physical Therapy.

Pay package based on 12-hour shifts and 36 hours per week (subject to confirmation) with tax‑free stipend amount to be determined. Posted job title: PT:
