- Collaborate with nurses, therapists, and physicians to optimize care outcomes
- Educate patients and caregivers on coping strategies and care plans
- Document all visits and interventions accurately in the EHREnsure compliance with home health, state, and federal regulations
- Participate in interdisciplinary case conferences and care reviews
- Support a patient-first, compassionate, and culturally sensitive care environment
