Hospice Chaplain

Peoples Home Health, LLC is seeking a compassionate Hospice Chaplain (PRN) to provide spiritual and emotional support to patients and families in their homes across the Pensacola area. In this patient-first, community-focused role, you will conduct spiritual assessments, offer counseling, prayer, and grief support, and collaborate closely with our interdisciplinary hospice team to align care with each patient's beliefs, values, and end-of-life goals.

You'll document visits, participate in care conferences, and help families navigate anticipatory grief and loss while upholding dignity, respect, and cultural sensitivity.

Responsibilities

• Provide spiritual and emotional support to hospice patients and families in their homes.
• Conduct spiritual assessments and integrate findings into individualized care plans.
• Offer counseling, prayer, rituals, and presence aligned with patient and family beliefs.
• Support families with anticipatory grief, bereavement, and end-of-life decision-making.
• Collaborate with the interdisciplinary hospice team to coordinate holistic care.
• Participate in care conferences and communicate changes in spiritual needs.
• Document visits, assessments, and interventions in accordance with agency policies.
• Respect diverse faiths, cultures, and values, ensuring inclusive, patient-centered care.
• Respond to urgent or crisis spiritual care needs as scheduled or requested.
• Educate patients, families, and staff on spiritual care and available community resources.
