Providence Hospice and Home Health is a growing division of Providence Life Services and is currently seeking a Hospice and Home Health CNA, Full time willing to travel to Crete/Lansing/Beecher/Chicago and surrounding areas.

The schedule is 32+ hours/week, Monday - Friday, with visits made during business hours, 8:00AM - 4:30PM. Mileage will be reimbursed at $0.76/mile, and the hourly rate range is $18.80 to $21.00.

• Assists with all ADL's including bathing (Activities of Daily Living), Medication Reminders, and Vitals, etc.
• Support and represent the mission of Providence as shown by your actions while on duty.
• Document changes in medical condition to the appropriate primary care nurse.
• Give and receive shift reports to ensure consistent medical care is delivered.
• Measure and record vital signs as necessary or instructed.
• Other duties as assigned.

• Must be an active CNA in the state of Illinois.
• Must have a compassionate, empathetic and nurturing personality.
• Home health and/or hospice experience strongly preferred.
• Comfortable traveling throughout the organizational geographic region.
• CPR certification (preferred)
• Current Driver’s license with good driving record
• Reliable transportation
