ABOUT THE ROLE

Responsible for meeting patient's priority needs by assessing the patient, participating in interdisciplinary care planning, providing and/or delegating direct patient care consistent with bedside nursing standards, including executing therapeutic physician-and nurse-initiated interventions, monitoring vital signs, administering specialty medications and chemo therapies that require specialized handling, unique administration and intensive patient monitoring, performing complex nursing treatments, and responding promptly to changes in patient conditions and assessing and reporting progress toward clinical outcomes.

Contributes to an efficient and effective work environment through professional communication, documentation, and coordination with other healthcare teams to achieve optimal patient outcomes and experience. Responsible for the appropriate delegation of nursing care to licensed and unlicensed personnel according to Nebraska statute.

For Cardiac Rehab:
Whenever possible, includes the family or significant others in the plan of care of the patient. Coordinates care and communicates essential information between professional disciplines, uses available resources and innovative strategies to provide care, empowers the patient and/or significant others to make informed changes in health‑related behaviors, promotes primary, secondary and tertiary prevention, and assesses tolerance of activity levels to adjust the program accordingly.

  • Collects and assesses patient data based on presenting needs and medical diagnosis, ensuring appropriate documentation.
  • Evaluates patient progress and communicates status and care plans with providers and care team members.
  • Determines care priorities and facilitates transitions to the next level of care as needed.
  • Utilizes resources and staff competencies to deliver safe, effective, and efficient care.
  • Assesses and addresses patient education and discharge needs, tailoring learning to individual needs.
  • Demonstrates leadership in the professional practice setting and in ambulatory nursing.
  • Performs hospital outpatient nursing skill set, including administration of specialty medication through intravenous infusions, injections and/or port or central line access, continuous monitoring for therapeutic response and potential drug reactions.
  • Performs blood product transfusions, central line access and maintenance, port-a-catheter access and maintenance, upholding infection prevention, medication safety and patient safety protocols.
  • Administers complex medications and treatments safely according to policy and provider orders, including hazardous medications and chemotherapeutic agents while providing continuous assessment and monitoring for adverse or toxic effects.
  • Provides comprehensive management of oncology patients, including laboratory assessment, medication education, infusion management, electrolyte replacement, injection administration and coordination of care with oncology nurse navigator, primary care provider and oncology/hematology provider team.
  • Educates patient on stress testing procedure, performs cardiac stress testing, collaborates with nuclear medicine technologists and cardiologists, and continuously observes and interprets EKGs, hemodynamic responses and patient status before, during and after the procedure.
  • Assists echocardiography technologist with diagnostic testing, including IV insertion, bubble studies and administration and documentation of intravenous contrast agents, ensuring patient comfort and procedural safety.
  • Makes accurate and timely observations and documents changes in the patient's condition in the chart according to policy.
  • Applies interpretation of diagnostic/monitoring data to the plan of care.
  • Assists with specialty provider multifaceted clinical procedures such as ultrasound guided injections, intra‑articular joint injections, microscopic ear exams, laryngoscope exams, gynecological outpatient testing, local anesthetic excision procedures, and independent nursing assessment of wounds with coordination of care with…
