Summary

The Patient Care Coordinator (“PCC”) coordinates/assigns field clinicians to patients. As a partner to our RN Case Managers and all disciplines on a team, the PCC coordinates schedules between multiple providers and disciplines to assure optimal patient care, efficient utilization of services, and compliance with physician orders. The PCC also serves as a partner to the business development team to ensure that admissions are properly managed and scheduled.

Responsibilities
  • Coordinates scheduling of home health patients with the assistance of the supervisor.
  • Responsible for maintaining the team master schedule and visit verification.
  • Updates direct care staff schedules weekly and as needed.
  • Performs visit verification activities daily. Verifies visits provided to visits ordered, scheduled, and documented.
  • Notifies the clinical manager of discrepancies between visits ordered, scheduled, and documented.
  • Coordinates with the Clinical Manager and business development team to schedule all patient admissions and initial visits multidisciplinary referrals.
  • Works under the direction of the Clinical Manager to rearrange schedules in situations of call-offs, conflict, or high utilization of service. Ensures prompt scheduling of emergent / as-needed visits as directed by the Clinical Manager.
  • Assures timely completion of individual staff schedules within established time parameters.
  • Accesses patient and provider schedules, authorizations, and physician orders to determine appropriate visit schedules under the direction of the Clinical Manager.
  • Assures efficient use of staff by utilizing full-time staff prior to scheduling part-time and per diem staff.
  • Coordinate, assign, and notify appropriate on-call staff and management of the on-call schedule.
  • Work alongside RN Case Managers and others to ensure that patient needs are addressed. This includes ordering supplies, communicating with patients, following up on patient requests, etc.
  • Work with the intake team and branch operations to make contact with new patients.
  • Complete other assigned duties consistent with the nature of this work.
Qualifications
  • Minimum of three years’ previous work experience, with a preference for home health experience.
  • LPN Licensure in state of practice strongly preferred.
  • Demonstrate a commitment to service to others and a willingness to help others in a team-based environment.
  • Knowledge of appropriate communication skills to include the ability to communicate with both internal and external customers and effectively present information one-on-one or in group settings.
  • Ability to solve problems involving many variables in both standard and exceptional situations.
  • Proficient in Excel and Word. Familiarity with other Office programs is a plus.
  • Must have excellent verbal and written communication skills.
