Role Overview

Seeking a Nurse Practitioner to provide in-home, comprehensive clinical care to high-risk patients within their residences, ensuring holistic health management without night, weekend, or holiday shifts.

• Conduct in-home assessments, including medical history, medication reconciliation, vital signs, and physical exams.
• Manage acute medical situations and chronic condition exacerbations with diagnostics and interventions.
• Coordinate care with an interdisciplinary team and document accurately, including ICD-10 coding.
• Engage in advanced care planning and provide patient and caregiver education and counseling.

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Unrestricted licensure in the applicable state and valid driver’s license with reliable transportation.
• Ability to adapt to varied environments and perform physical duties as needed.
• Preferred: 1+ years clinical experience in home health, geriatrics, or long-term care, proficiency with EMR systems, and experience addressing complex social and behavioral needs.

Competitive salary based on experience, ranging from $109,500 to $164,000 annually, with comprehensive benefits, incentives, and growth opportunities.
