The Registered Nurse (RN) provides and directs the provision of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations and agency policies. The RN coordinates the total plan of care with other health care professionals, ensuring continuous patient care by planning and exchanging information with physicians, agency personnel, patient, family, and community resources.
Responsibilities- Makes the initial and/or comprehensive nursing evaluation visit, determines the primary focus of care, develops the plan of care within state-specific guidelines, and submits accurate, complete, and timely documentation, per policy.
- Conducts home visits to clients in designated geographic territories.
- Initiates, develops, implements, and makes necessary revisions to the plan of care in collaboration with the physician and other health care professionals involved in care.
- Communicates current health status and needs of clients to the appropriate staff.
- Provides on-call coverage and responds to calls.
- Documents and completes POC for errors/findings.
- Completes initial assessments and aide assignment sheets for client personal/attendant care.
- Ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source.
- Audits/reviews medication logs.
- Current RN licensure in state of practice.
- Current CPR certification required.
- Registered Nurse 3 years of experience (Preferred).
- Current driver license and vehicle insurance.
- Must be 18 years of age or older.
- Must be able to pass a TB skin test and drug screen.
- Must be able to pass a criminal history check.
