Primary Responsibilities
- Provides high quality clinical services within the scope of practice, as defined by state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other health‑care team members, following all applicable infection control procedures.
- Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency's electronic visit verification program (as applicable).
- Actively participates in the agency's care coordination process, including timely reporting of patients' needs and changes in condition, attending case conferences, and communicating effectively with patients, family, physicians, clinicians, agency management staff, case coordinators, and community case managers (as applicable).
- Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
- Current CPR certification.
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
- LA:
- Current LPN licensure in LA without restrictions.
- One or more years of experience working as an LPN.
- Home care experience.
- Ability to work flexible hours as required to meet identified client needs.
- Ability to manage multiple tasks simultaneously.
- Able to work independently.
- Good communication, writing, and organizational skills.
Pay is based on local labor markets, education, work experience, certifications, and other factors. In addition to salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (subject to eligibility). The hourly pay for this role ranges from $21.70 to $32.55 per hour for full‑time employment. We comply with all applicable minimum wage laws.
