The Province of Our Mother of Good Counsel of the Order of St. Augustine seeks a full-time in-person Health Care Coordinator (HCC) for its Crown Point, IN facility. The HCC will organize care services, support Augustinians’ well-being, and serve as a liaison among province administration, health care staff, residents, their families, and professionals. The HCC will report to the designated on-site Augustinian Supervisor.
Key Responsibilities- Attend weekly meetings with the Augustinian Health Care Team.
- Document client doctor visits, specifically matters related to medications, tests, prognoses, and upcoming appointments.
- Develop, maintain, and update database with current information for patient records and care plans.
- Report regularly on patient status and program progress.
- Join provincial administration meetings with the Health Care Team.
- Visit Augustinian residents on a weekly basis to stay updated on any changing needs.
- Serve as liaison between Augustinians and health care facilities.
- Handle intake and orientation of new Augustinian residents.
- Update family members on care plans.
- Communicate and coordinate with off-site Assessor of Augustinians for general health-related issues.
- Develop and coordinate transition plans for Augustinians moving to health care facilities.
- Other related job responsibilities as needed and assigned by on-site Augustinian Supervisor.
- Travel required—must have reliable transportation.
- Demonstrates effective organizational and record-keeping skills, as well as sufficient technological skills to maintain and update health care database.
- Effectively communicates and has good people skills.
- Works collaboratively as part of a multidisciplinary team.
- Maintains a patient-oriented approach.
- Experience:
Health care or case management experience is a plus. - Annual salary range: $60-80,000 (actual salary will be based on experience and background)
