What You'll Do
  • Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
  • Build and adjust personalized care plans in collaboration with physicians and care teams.
  • Administer medications and treatments per physician orders.
  • Keep clinical documentation accurate, timely, and meaningful.
  • Serve as the communication bridge between families, physicians, and community health partners.
  • Educate patients and caregivers so they feel confident and supported.
  • Participate in on‑call rotation per team policy.

Requirements
  • Active RN license in the state of employment.
  • Graduate of an accredited school of Nursing.
  • One (1)+ year hospice, home health or related organization experience preferred.
  • Compassion for patients and families navigating end‑of‑life care.
  • Strong clinical judgment and the ability to work independently in the field.
  • Excellent communication skills across care teams and families.
