Transitions Hospice is seeking a Care Coordinator to grow relationships and referrals in your assigned territory in Indiana. You will be the face of our organization in the community, building trust with physicians, discharge planners and care facilities to ensure patients receive timely hospice services.
The role requires 3+ years in healthcare sales, excellent communication, and the ability to travel locally. A Bachelor’s degree and healthcare knowledge are preferred.
