Manage patient access to healthcare and connect individuals with community resources.

Critical Information Work schedules will be created in 6-week blocks following 3 weeks of training. Shifts will be between 7-10 hours during clinic operating hours. A typical work week will be two or three weekdays and every other weekend. This is an in-person, patient-facing role.

• LPN/LVN license must be full, active, and unrestricted in the state of employment.
• Previous experience in Primary Care, Urgent Care, or Emergency Room settings.
• Proficiency with electronic medical record (EMR) systems;
  Epic experience preferred.
