Wraparound Coordinator

The Wraparound Coordinator partners with families and referral sources to develop and lead a Child and Family Team using a strengths-based, trauma-informed approach. This role ensures implementation of individualized care plans aligned with the National Wraparound Initiative Model, supporting families in achieving their identified goals.

• Facilitate Child and Family Team development, meetings, and ongoing collaboration.
• Lead the creation, implementation, and evaluation of individualized, strengths-based service plans.
• Identify barriers, develop solutions, and ensure fidelity to the Wraparound model.
• Support families in engaging natural supports and community resources.
• Complete required documentation and ensure compliance with confidentiality and program standards.

• Bachelor's degree in Social Work or related Human Services field.
• Experience in case management, social services, or child welfare preferred.
• Strong communication, organization, and problem-solving skills.
• Ability to work flexible hours, including evenings and occasional weekends.
• Valid driver's license, insurance, and ability to pass background screenings.
