Compensation:

$32-$38/ hr based on licensure and experience

  • Key Responsibilities
  • Conduct and coordinate comprehensive assessments of new and existing residents using the MDS tool.
  • Develop, implement, and review individualized care plans in collaboration with an interdisciplinary team.
  • Ensure compliance with federal and state regulations for MDS assessments and care planning.
  • Monitor and transmit MDS assessments to the relevant authorities.
  • Train and guide nursing staff on MDS procedures and updates.
  • Audit clinical records to ensure accuracy, completeness, and regulatory compliance.
  • Collaborate with healthcare providers to optimize resident care outcomes.
  • Qualifications
  • Active RN license required.
  • MDS Certification preferred.
  • Proven experience in clinical assessment and MDS coordination.
  • Strong knowledge of federal and state regulations related to MDS and care planning.
  • Exceptional organizational skills and attention to detail.
  • Excellent communication and interpersonal skills to interact with staff and residents.
