Bright Spring Health Services is seeking a Travel Physical Therapist for a 13-week Home Health contract in Cullman, Alabama

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on each patient's functional goals.
• Treat patients with orthopedic, neurological, post-surgical, and medically complex conditions in the home setting.
• Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
• Collaborate with physicians, nurses, case managers, and the interdisciplinary care team.
• Educate patients and caregivers on home exercise programs and safety strategies.
• Complete timely EMR documentation and progress reporting.
