Job SummaryA licensed practical or vocational nurse (LPN/LVN) provides skilled nursing care in patients’ homes on an intermittent basis, following physician orders and care plans under the supervision of a registered nurse.
Responsibilities- Understand and adhere to established policies and procedures.
- Implement the nursing care plan for each patient.
- Provide nursing services, treatments, and diagnostic and preventive procedures as assigned.
- Initiate preventive and rehabilitative nursing procedures as appropriate.
- Observe signs and symptoms and report to the physician and RN, including reactions to treatments, medications, and changes in the patient’s physical or emotional condition.
- Teach and counsel the patient and family/significant others regarding nursing care needs and related problems.
- Evaluate, with the registered nurse, the effectiveness of the LPN’s/LVN’s nursing service to the patient and family under the guidance of the RN.
- Maintain accurate and complete records of observations, treatments, and care of patients.
- Participate in medical record audit as assigned.
- Attend staff meetings, patient care conferences, and in‑services as scheduled.
- Take on‑call duty, nights, weekends, and holidays as assigned.
- Submit any schedule changes to the Director of Clinical Services/Nursing Supervisor on a daily basis.
- Participate in patient care conferences to discuss the need for involvement of other health team members (e.g., physical therapist, speech‑language pathologist).
- Prepare clinical and progress notes.
- Assist the physician and RN in performing specialized procedures.
- Prepare equipment and materials for treatments.
- Assist the patient in learning appropriate self‑care techniques.
- Graduate of a state‑approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing.
- Minimum of one year experience in nursing (preferred).
- Acceptance of the philosophy and goals of this agency.
- Ability to exercise initiative and independent judgment.
Works indoors in agency offices and patient homes and travels to/from patient homes.
Job RelationshipsSupervised by:
Director of Clinical Services, Nursing Supervisor, RNs.

Lifting RequirementsAbility to perform the following tasks if necessary:
- Participate in physical activity.
- Work for extended periods of time while standing and being involved in physical activity.
- Heavy lifting.
- Do extensive bending, lifting, and standing on a regular basis.
- BLS
