General:

Defined:

The Ryan White Outreach Coordinator is responsible for HIV outreach, testing, and linkage-tocare activities for populations at high risk for HIV infection in the Fredericksburg area region, as well as the day-to-day administration of FAHASS's 340B Drug Pricing Program. This position bridges community-based outreach with clinical program compliance, ensuring that newly diagnosed individuals, those out of care, and eligible patients are connected to - and retained in - Mosaic Care Center and Ryan White-funded services.

The Ryan White Outreach Coordinator works in close collaboration with the clinical team, case managers, and community partners to advance health equity and the FAHASS mission.

Employment Standards:
  • High school diploma or General Education Development (GED) required;
    Associate's or Bachelor's degree in Public Health, Health Education, Social Work, or a related human services field preferred.
  • Minimum of one (1) year of experience in HIV outreach, community health work, case management, or a related field.
  • Virginia Department of Health (VDH)-approved HIV Counselor certification required, or willingness and ability to obtain certification within 90 days of hire.
  • Experience with 340B program administration, pharmacy operations, or healthcare compliance strongly preferred.
  • Familiarity with Ryan White Part B program requirements, eligibility standards, and service categories preferred.
  • Experience working with people living with HIV/AIDS, LGBTQ+ individuals, or other medically underserved communities preferred.
  • Demonstrated ability to conduct outreach and build trusting relationships with individuals from diverse socioeconomic, cultural, and linguistic backgrounds.
  • Bilingual (English/Spanish) a benefit.
Key Responsibilities Include the Following: HIV Outreach & Testing
  • Conduct HIV screening and testing using approved rapid testing protocols and VDH guidelines, including pre- and post-test counseling consistent with Virginia HIV testing laws.
  • Plan, organize, and facilitate HIV testing events targeting populations at high risk for HIV infection, including MSM, transgender individuals, people who inject drugs (PWID), and communities of color.
  • Identify, locate, and engage newly diagnosed HIV-positive individuals and those who have fallen out of care, facilitating prompt linkage to Mosaic Care Center and Ryan White services.
  • Conduct active field-based outreach at community venues, shelters, harm reduction sites, LGBTQ+ spaces, health fairs, and other locations where at-risk individuals congregate.
  • Attend and represent FAHASS at community events, partner meetings, coalitions, and health fairs to promote HIV awareness, prevention, and available services.
  • Distribute HIV prevention materials, condoms, and information on PrEP, PEP, and harm reduction resources during outreach activities.
  • Provide basic health education and information on sexual health, STI prevention, and linkage to additional FAHASS services.
  • Coordinate STI screening and referral in conjunction with HIV testing activities, as applicable.
Ryan White Part B Outreach Services
  • Provide Ryan White-funded outreach services consistent with Virginia's Part B service category definitions, including:
    • Identifying and making contact with HIV-positive individuals who are not in care or not receiving Ryan White services.
    • Providing information about and facilitating access to Ryan White-funded medical and support services.
    • Following up with individuals who miss scheduled medical appointments to support re-engagement in care.
    • Assisting individuals with navigation of the healthcare system, including explaining eligibility requirements and available service categories.
    • Coordinating with medical case managers to facilitate warm handoffs and intake for newly linked clients.
    • Documenting all outreach contacts, encounters, and referrals in CAREWare or the designated data system per Ryan White reporting requirements.
