Job SummaryThe RN Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive/complex/safe discharge plans. The RN Case Manager collaborates with MSW for guidance with psychosocially complex cases and facilitates coordination of care. They are a resource for discharge planning and care coordination for medically complex cases and maintain knowledge of available community resources/post‑acute care options.
Responsibilities- Lead the discharge planning process for complex cases.
- Collaborate with physicians, nurses, social workers, and other team members to develop and implement discharge plans.
- Coordinate post‑acute care and community resources.
- Assess patients’ needs and determine appropriate care transitions.
- Monitor quality and safety of core processes in the department.
- Communicate regularly with staff and patients to improve processes.
- Perform medical tasks under the direction of practice physicians, including history and physical, labs, diagnostics, and independent evaluation during emergencies.
- Provide monitoring and continuity of care between physician visits.
- Anything else as assigned.
- Education: Associate's Degree in Nursing required. Preferred: 4‑Year/Bachelor's Degree, BSN.
- Experience: 2 or more years of healthcare experience preferred; case management or related experience preferred.
- Licensure: Current RN licensure in the state of employment required.
- Additional Skills/Requirements: Weekend and holiday coverage; preferably competent in Microsoft Office.
- Patient Care
- Interview and advise patients regarding health and illness prevention.
- Synthesizes data to determine preliminary diagnosis and therapeutic plan using prevention principles.
- Recommends community resources to meet patient and family needs.
- Quality
- Collaborate with other departments to achieve desired quality outcomes.
- Monitor quality of core processes.
- Apply principles of safety and infection control.
- Communication
- Demonstrate superior interpersonal and communication skills.
- Work within a team to achieve patient and team goals.
- Share and initiate regular professional communication with co‑workers.
- Participate in regular staff meetings.
- Identify opportunities to improve processes based on staff and customer feedback.
- Medical Knowledge
- Obtain complete history and physical data.
- Review patient records.
- Interpret and integrate data for diagnostics and therapeutics.
- Perform a complete physical exam and record findings.
- Establish presumptive diagnosis.
- Order appropriate laboratory and diagnostic procedures.
- Perform independent evaluations and treatments in emergency situations.
- Provide monitoring and continuity of care between visits.
Medium to Heavy Physical Demand: constant standing, frequent walking, bending/stooping; requires proficient communication, auditory, and visual skills; attention to detail; ability to lift/push/pull 100 lbs. Use Safe Handling Equipment for weights over 35 lbs. The job may not include all assigned duties and can be amended at UVA Community Health’s discretion.
Other- May require the use of safety equipment such as HEPA mask for infection prevention.
- On‑call responsibilities as directed.
- Ability to travel between campus buildings, remote facilities, and out of town as needed.
The base compensation range for this role is $31.22 – $50.50 hourly. Individual compensation will be determined by the selected candidate’s qualifications, previous work experience, and/or education. The pay range does not include incentive compensation programs such as shift differentials and clinical ladders that may be available for eligible positions.
