Licensed Practical Nurse - Medical Group Job Summary

The Licensed Practical Nurse is responsible for providing direct patient care, ongoing education and reinforcement of care plans, and general support in an ambulatory setting under the direction and supervision of the patient's physician or licensed medical practitioner. The position requires providing services to all age populations in a manner that demonstrates an understanding of the functional/developmental age of the individuals served.

Responsibilities
  • Practice safely within the defined standards of practice:
    • Record and relay patient information, symptoms, and concerns to the patient's medical provider or registered nurse.
    • Assist patients with therapeutic regimens.
    • Reinforce established plans of care and age-appropriate interventions.
    • Reinforce patient/family teaching plan based on identified health education needs.
    • Administer tasks, treatments, and medications in accordance with validated competencies.
  • Communicate effectively and professionally with all members of the interdisciplinary team, patients, medical providers, and all levels of healthcare:
    • Convey information accurately to physicians, nurses, and other team members and listen and respond to the needs and concerns of patients and colleagues.
    • Utilize appropriate communication tools within the interdisciplinary team.
    • Document patient information in EHR.
  • Show teamwork and support for teammates to achieve common goals:
    • Organize and prioritize daily work assignments to support patient needs and operational efficiency.
    • Demonstrate teamwork in the delivery of patient care and recognize the importance of time-sensitive issues.
  • Practice within the boundaries of nursing regulations in Virginia:
    • Engage in professional development activities to stay current with advancements and best practices in ambulatory care.
    • Possess clinical knowledge and skills to meet standards required by specific clinical areas.
    • Contribute to the development of practice guiding resources such as policies, procedures, guidelines, and protocol orders.
  • Incumbent may be asked to perform additional duties as assigned.
Qualifications
  • Education: High School Diploma required. Graduate of an accredited Practical Nursing Program required. Completion of a nursing program approved by the State Board of Nursing.
  • Experience: Minimum of 1 year of nursing experience preferred.
  • Licensure: Licensed to practice as a Practical Nurse (LPN) in the Commonwealth of Virginia prior to onboarding.
  • Skills & Requirements:
    • AHA Healthcare Provider BLS required.
    • Ability to successfully complete general and department role-based specific skills validation and competency testing.
  • Preferred: Not specified.
Physical Demands

Medium to Heavy Physical Demand:
The job requires constant standing, frequently walking, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.   • Base compensation range: $22.31 - $34.37 hourly. Compensation will be determined by qualifications, experience, and education. The pay range does not include incentive compensation programs such as shift differentials, clinical ladders, and other incentives that may be available for eligible positions.
