• Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families.
• Sets measurable objectives formulated in conjunction with the rehab team.
• Assists the physician and other health team members in understanding significant social and emotional factors related to patient’s health problems.
• Instructs and counsel patients and families in treating and coping with social and emotional responses connected with illnesses.
• Participates in care coordination activities and acts as a resource to other health team members in identifying and resolving patient needs.
• Identifies and assists the patient and/or family in utilizing appropriate community resources to achieve measurable objectives.

• Master’s Degree in Clinical Social Work (LCSW).
• Current Licensed Clinical Social Worker Licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis.
• Current BLS (Basic Life Support), First Aid, and CPR certification required.
• Ability to lift 50 pounds; moving, lifting, or transferring patients up to 100 pounds following safety procedures.
• Fine motor skills, visual acuity, and ability to walk or stand for extended periods.
• Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently.

It is designed to help people who have suffered brain injury, spinal cord injury or stroke/CVA regain functional capabilities. By providing care in the home, community, workplace, or school, we promote greater health and dramatically improve functional skills and independence for patients.

Salary Range: USD $60.00 - $65.00 per hour.
