Roles & Responsibilities Perform comprehensive patient evaluations to assess functional performance and rehabilitation needs
Develop and implement personalized treatment plans aligned with patient goals
Deliver therapeutic activities, exercises, and adaptive strategies to maximize independence
Monitor and document patient progress while ensuring compliance with state and federal guidelines
Educate patients and families on therapy goals, adaptive equipment, and home programs
Collaborate with PTs, nurses, physicians, speech therapists and the interdisciplinary team to coordinate excellent care
Qualifications Active and current Occupational Therapist (OT) license in the state of practice (required)
Graduate of an accredited Occupational Therapy program (OTD or equivalent)
Previous experience working in a Skilled Nursing Facility (SNF) is required
Strong clinical knowledge with a patient-centered approach
Excellent communication, organization, and teamwork skills
Education OTD or equivalent — graduate of an accredited Occupational Therapy program
Knowledge & Skills Skilled in patient evaluation and treatment planning
Expertise in therapeutic activities and adaptive strategies
Clear, compassionate patient and family education
Strong teamwork and interdisciplinary collaboration
Effective organization and clinical documentation practices
Ability to apply evidence-based OT interventions to improve daily living independence
