Nurse Practitioner – Skilled Nursing Facility – Western, Maryland
1 hour west of Hagerstown and 2 hours west of Baltimore.
Provide patient-centered care in a skilled nursing facility where you will join a collaborative team focused on improving the quality of life for both short- and long-term residents through compassionate, evidence-based care.

- Be able to do the job as described
- Board Certified Nurse Practitioner
- Ability to work in a skilled nursing or post-acute setting
- Strong communication skills with patients, families, and care teams
- Maryland NP license
- Serve as primary care provider for skilled nursing residents
- Perform physical exams, assessments, and medical history reviews
- Develop and implement patient-centered care plans
- Collaborate with physicians and interdisciplinary staff
- Educate residents and families on treatment plans
