Located in Cumberland, MD - UPMC Western Maryland is full of opportunities for those looking to take the next step in their career!  We are currently hiring an RN to join our Hospice team!

This is a full-time, 40 hour/week position, working dayshift. Includes rotating weekends, holidays, and an on-call rotation.

_Applicants will be placed in the appropriate job title and salary from Career Ladder based on their individual experience and education. Title and salary will be determined based upon education and nursing experience. Pay rates shown are for the Professional Staff Nurse, HH/Hspc job title. Previous nursing experience is preferred but not required._

The Professional Staff Nurse is responsible for the delivery of skilled nursing services to patients in their homes according to the physician's plan of care, agency policies and procedures, fiscal intermediaries, regulatory agencies and bodies, and within recognized standards of nursing practice. Must possess the ability to assess and interpret patient data needed to identify each patient's requirements relative to the population served.

Establishes and maintains collaborative relationships with physicians, other health care providers, patients, and their families, to achieve desired patient outcomes throughout the continuum of care. Assignments will be determined by the Clinical Manager of the branch based on a number of different factors, which include, but are not limited to, coverage for Leave of Absences, increase in patient volume in a specific team, and vacancies.

** Responsibilities:*
* + Coordinates case management activities within an interdisciplinary health care team to ensure appropriate delivery of services to patient. Attends compliance training and adheres to the organization standards of conduct, policies and procedures. Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. Completes an initial and comprehensive assessment of patient.

+ Constructively receives feedback and direction. Takes action to improve knowledge, skills and performance based on feedback or on self-identified developmental needs. Initiates, coordinates and ensures ongoing communication with the physician and other members of the interdisciplinary health care team, including evaluating patients and family centered health teaching. Provides direct patient care, evaluates outcomes and adjusts nursing care process as indicated to ensure optimal patient care.

Establishes the plan of care in collaboration with the physician, patient, and interdisciplinary team members in accordance with the physician orders and patient needs.

+ Assigns other interdisciplinary team members as appropriate to assist in the appropriate delivery of services as ordered on the plan of care to the patient. Supervises and teaches other nursing personnel including LPNs and Home Health Aides as needed. Attends and participates in case conferences, team meetings, and other work-related meetings. Recognizes limitations and requests assistance and direction to meet performance goals.

Completes all necessary documentation which may include visit assessment, plan of treatment, OASIS, 485 and Verbal Orders accurately and promptly in electronic documentation system while in the patient's home and within the agency's and regulatory standards. Data syncs (transmits) information same day.

+ Identifies, investigates and recommends solutions to organization issues.

Exercises independent judgement in matters concerning emergent and non-emergent patient care needs and communicates with the physician.

Identifies, develops and participates in process improvement opportunities within the agency that will enhance the quality of the services we provide.

Participates in weekend and on-call coverage rotation for the agency as per BU guidelines.

Demonstrates knowledge and understanding of the UPMC standard of care delivery and proficiency in all aspects of patient care, to include complex and/or specialized care.

+ The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below. They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures.

+ 6 months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care preferred.

+ BSN preferred.

+ Ability to establish and maintain positive, caring relationships with executives, managers, physicians,…
