Responsibilities
- Initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living.
- Implementation of the individualized plan of care and recommendation of revisions as necessary, and planning for discharge/transfer as appropriate.
- Consultation with and education of the patient/family/caregiver regarding the disease process and teaching self‑care techniques, as well as providing medication, diet and other instructions as ordered by the physician.
- Initiation of appropriate preventive and rehabilitative nursing procedures.
- Collaboration in the coordination of all patient/family services and prioritization of needs with the interdisciplinary team to develop and revise the patient’s individualized plan of care.
- Communication of changes in the patient’s needs and outcomes to the physician and other personnel, updating the Plan of Care as needed.
- Collaboration to determine scope and frequency of services based on acuity and patient/family needs.
- Coordination of care with interdisciplinary team members and the family/caregiver to meet identified needs, including psychosocial, spiritual and emotional needs and referral to appropriate disciplines/services.
- Strong desire to join an organization committed to providing the best patient‑centered care.
- Graduate of an accredited nursing school; LPN required, BSN preferred.
- Licensed as a Practical Nurse in Rhode Island and Massachusetts.
- Minimum of 1–3 years of clinical experience.
- Excellent assessment, clinical and communication skills.
- Bilingual preferred but not required.
