Job Highlights
Each visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and optional diagnostic tests such as diabetic eye exams or spirometry. The clinician uses an iPad and a clinical workflow for each evaluation. The role does not require prescribing medication, ordering lab tests, or altering existing regimens.
In this role, you will:- Bring your heart into every visit as part of a national network of purpose‑driven clinicians dedicated to improving health outcomes.
- Visit millions of people nationwide to connect them with the right care for their unique needs.
- Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
- Be flexible to travel locally and within licensed states, as needed.
- Conduct virtual visits as needed.
- Provide all Signify Health services indicated, including diagnostic & preventive services such as spirometry, DEE, and bone density screening.
As an in‑home health clinician, your primary workplace is the private residence of the member. Environments vary and may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be comfortable and safe performing duties in these settings.

Preferred Qualifications (NPs and PAs)
- Active, unrestricted license(s) in the coverage area(s).
- Board certification (required).
- Multi‑state licenses (preferred).
- Able to cover Rhode Island.
Anticipated weekly hours: 29.5.
Time type:
Part time.
Pay range: $46.03 – $99.14 per base hourly rate (dependent on experience, education, geography, and other factors). Additional reimbursement opportunities may be available.
