Continuum Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our nursing team providing end-of-life care throughout the community.
Schedule:
Mon-Fri, 8 am - 4:30 pm with on-call rotation. Territory:
Warwick/Coventry, RI.
You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through their hospice journey with compassion, clinical excellence, and connection.
