LOCATION: North & Rotating Health Centers

DIVISION/DEPARTMENT: Clinical Services

SALARY INFORMATION: $20.00 Hourly / Pay Grade TPW / Job Code T1001

Participate in scheduled community outreach events providing prevention education, screening support, and navigation services in clinics, mobile units, and community-based settings throughout DeKalb County. Deliver patient-centered health education, prevention counseling, and referral support consistent with public health guidelines and established protocols. Assist with specimen collection and point-of-care screening activities following required training and certification. Support linkage to care and prevention services, including referrals to follow-up services, treatment programs, and preventive services.

Prepare outreach materials, supplies, and documentation prior to events to ensure operational readiness. Utilize electronic data collection systems and/or paper documentation tools to accurately record outreach encounters and services provided. Collect, secure, process, and organize confidential records from outreach events and clinic locations in compliance with privacy, HIPAA, and data security requirements. Track service volumes, referrals, processed forms, and outreach metrics in accordance with program reporting standards.

Conduct routine inventory audits of outreach supplies and report replenishment needs to supervisory staff. Operate county or program vehicles to support mobile outreach services; may be required to use personal vehicle with mileage reimbursement. Collaborate with supervisors, clinicians, data staff, prevention team members, and other public health personnel to achieve program objectives. Maintain readiness to work evenings, weekends, and non-traditional hours as required for outreach events.

Perform other duties as assigned to support DeKalb Public Health programs and services.

Bachelor's degree in education, social, behavioral, or biological sciences or a directly related field from an accredited college or university. Minimum of one (1) year of experience in community outreach, patient navigation, healthcare support, public health, or related human services field. Ability to communicate effectively with individuals from diverse cultural, social, and economic backgrounds, including close collaboration with members of the LGBTQ+ community without bias or prejudice.

Ability and willingness to obtain required certifications and training related to public health screening, specimen collection, and outreach operations. Ability to provide basic health education, prevention information, and supportive counseling in accordance with public health guidelines. Experience with or ability to learn electronic data entry systems, tablets, and public health documentation tools. Ability to accurately complete, process, and maintain confidential paper and electronic records in compliance with privacy and data security requirements.

Strong organizational and time management skills with the ability to manage multiple priorities and meet deadlines. Valid driver’s license and ability to operate county or program vehicles as required. Ability to travel throughout DeKalb County to support outreach services. Ability to work independently while functioning effectively as part of a multidisciplinary public health team. Ability to lift and carry outreach supplies and equipment weighing at least 25 pounds.

Ability to stand, walk, bend, and perform repetitive tasks for extended periods during outreach events.
