The Southeast Health District, Wellness program is seeking a highly qualified candidate to fill the full-time, salaried position of Social Service Specialist based in Statesboro, GA, Waycross, GA or Douglas, GA.

Under general supervision, performs a variety of case management services for the housing program. Duties to include but not limited to completing housing assessments, assisting clients with developing and implementing permanent housing plans, maintaining files and EHR, providing case conferencing and referrals, assisting in developing community partnerships and resources, and completing reports on housing outcomes.
QualificationsMaster's degree in a related field from an accredited college or university AND eligibility to be licensed OR Bachelor's degree in a related field from an accredited college or university AND Two years of experience in social service delivery.
Preferred QualificationsMaster's degree in a related field from an accredited college or university AND Two years of experience in social service delivery. Experience working with HIV/AIDS patients.
Employment InformationCurrent State employees are subject to State Personnel Board rules regarding salary. Southeast Health District accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered. SEHD will contact educational institutions to verify degree, diploma, licensure, etc. The candidate selected for this position may be subject to pre-employment drug screening and a criminal background check.
As an employee of SEHD, in the event of an identified emergency you may be required, as a term and condition of employment, to assist in meeting the emergency responsibilities of the department.
