Falls Village Skilled Rehabilitation and Wellness is a 103 bed facility with all private rooms located in Cuyahoga Falls, OH.

Sign on Bonus- $7500.00 for full time status, $3750.00 for part time status, paid out in quarterly installments if eligible.

$3.00 shift diff for 2nd and 3rd shifts!!!

LPN’s will provide quality nursing care to our patients and family needs; working under the direction of the Care Manager, RN. The care setting is a skilled nursing care facility.

• Registered LPN with Ohio License and a minimum of one year of LPN experience
• Valid Ohio Driver’s License
• Competency in information technology
• Evaluation skills sufficient to report to RN Case Manager clinical details to support patient needs and goals
• Documentation skills sufficient to record patient/family
• Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals
• Ability to write routine reports and correspondence
