• Provide direct care to all assigned patients, managing communication with care team members (pharmacists, prescribers, dietitians, home patient representatives, care partners, and community resource staff).
• Evaluate and educate patients/care partners to safely administer home infusion therapy and provide additional instruction as needed.
• Assess the home environment to identify interventions required for safety and appropriateness of care.
• Develop and evaluate the plan of care in collaboration with other team members, monitoring response to therapy.
• Manage schedule via the company‑approved platform per policy and communicate any changes promptly.
• Coordinate patient care through the approved company platform.
• Participate in the nurse on‑call rotation to ensure continuous support for customers.
• Document all patient encounters, communications, and care activities at the time of service per policy and regulatory requirements.
• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
• Submit plans of treatment (POT) and required clinical documents promptly per policy and state guidelines.
• Collaborate with departments to ensure all required patient documents (e.g., consents, POTs) are completed and returned to the branch office within an acceptable timeframe.

• Part‑time, 30–32 hours per week, Monday to Friday, with willingness to work one Saturday per month.
• May participate in an on‑call rotation.
• Must be knowledgeable of IV access devices, including IV lines, ports, and infusion medication administration.
• Must be able to drive up to 100 miles round trip as needed (mileage reimbursement and hourly rate paid).
• Must be licensed as an RN in the state of Texas.

• Current RN license in good standing.
• RN degree; bachelor’s degree in nursing (BSN) from an accredited institution preferred.
• Advanced certification in specialty areas (e.g., CRNI, ONS, IgRN) preferred but not required.
• Participation in on‑call rotation, nights, weekends, and holidays as needed.
• Administrative oversight experience—monitoring workflows, ensuring compliance, and managing resources.
• Rapid emergency response capability during non‑standard hours.
• Minimum 3 years of nursing experience, preference for infusion therapy.
• Experience in ICU and/or cardiac care preferred.
• Valid state‑issued driver’s license with no significant restrictions.
• Proficiency with Microsoft Outlook and Office Suite.
