Case Manager Coordinator

Houston Methodist seeks a Case Manager Coordinator to support safe, patient-centered care transitions across our leading healthcare system. In this role, you will assess patient needs, develop and coordinate individualized care plans, and facilitate discharge planning and referrals to community resources. You will collaborate closely with physicians, nurses, social workers, and ancillary teams to optimize outcomes, reduce readmissions, and ensure regulatory compliance.

Responsibilities

• Coordinate comprehensive case management services for patients across the continuum of care
• Conduct assessments, develop care plans, and monitor patient progress and outcomes
• Collaborate with physicians, nurses, social workers, and other interdisciplinary team members
• Facilitate referrals to community resources, post-acute care, and support services
• Educate patients and families on care plans, treatment options, and discharge instructions
• Track utilization, document in EMR, and ensure regulatory and policy compliance
• Identify barriers to care and support safe, timely discharge planning
• Participate in quality improvement initiatives and case review meetings
• Maintain accurate, timely documentation and reporting metrics
• Support a culture of safety, compassion, and patient-centered care
