About the Role:
RN Case Manager (RNCM)
Make a difference beyond the bedside.
As an RN Case Manager, you'll support patients and families throughout the hospital stay and into the next phase of care. This is a highly autonomous role for a seasoned acute care Case Manager who thrives on collaboration, critical thinking, and advocacy across the full continuum of care. You'll partner closely with physicians, nurses, utilization review, and the interdisciplinary team to ensure care is coordinated, efficient, and centered on patient needs-because a 5‑star hospital deserves 5‑star Case Managers.
Schedule:
Monday-Friday, 8:00 a.m.
-5:00 p.m.
(Weekend rotation as required)
What You'll Do
* Independently assess patient and family needs and develop a comprehensive, coordinated plan of care
* Support safe, timely, and smooth transitions of care by connecting patients with the appropriate next level of care and community resources
* Actively participate in interdisciplinary rounds and proactively remove barriers to patient progress and discharge
* Manage length of stay by ensuring the right services are delivered at the right time
* Partner with community providers to coordinate post-acute and post-hospital services
* Support regulatory standards, National Patient Safety Goals, and patient safety initiatives
* Provide cross-coverage across RN Case Management service lines as needed
* Serve as a collaborative team player while functioning confidently and autonomously
Who You Are
You are a progressive thinker and experienced RN Case Manager who:
* Brings strong acute care and discharge planning expertise
* Is vulnerable and receptive to feedback, using it as a tool for growth
* Approaches challenges with flexibility, professionalism, and innovation
* Lives core values of patient advocacy, accountability, and collaboration
* Takes pride in delivering 5-star case management in a 5-star hospital
Minimum Qualifications
* Active Georgia RN License
* Associate Degree in Nursing (from an accredited program)
* 3-5 years of direct patient care and/or case management experience
* Demonstrated experience with financial management and discharge planning
Preferred Qualifications
* CCM or ACM certification
* Bachelor's Degree (BSN or related field)
* Previous acute care case management experience in a complex healthcare environment
