Provide outpatient geriatric evaluations and management of chronic and complex conditions Develop and implement preventive and primary care strategies for elderly patients Coordinate care plans involving multiple health services to improve patient outcomes Document patient visits accurately and efficiently using Epic EMR Offer education and counseling on health maintenance and disease prevention Manage post-acute and ongoing health issues within a clinic setting Collaborate with interdisciplinary team members to optimize patient care What You Need:

Required:

Board Certification in Internal Medicine or Family Medicine Minimum 1 year of outpatient clinical experience in geriatrics or primary care Valid Texas medical license or eligibility for licensure Preferred:
Experience managing chronic conditions common among geriatric patients Strong communication and interdisciplinary collaboration skills About the Assignment:
