Job Description
We are seeking the right candidate to join our team as a Case Manager! This fast-paced career involves working with a diverse population of clients with special needs and their families while managing their lives in the form of case management. The Case Manager would also be responsible for maintaining various programs that work with the Department of Family and Protective Services and Health and Human Services.
The right candidate will essentially be the Director’s Alternate Representative. A brief job description is as follows:
Responsibilities- Meeting with participants face‑to‑face regularly as required by government contractual obligations
- Conduct and participate in regular meetings
- Develop and maintain accurate and complete case records
- Assists eligible recipients to gain access to needed medical, behavioral, health, social, educational, and other services.
- Create, Initiate, and Modify the client’s Individual Service Plan and Methodologies based on the Client's Needs
- Educate patients and families
- Respect each patient’s rights during the provision of care or service and conduct business in an ethical manner
- Positively represent the company by always maintaining a professional and courteous manner
- Ability to interpret electronic monitoring messages and daily summary reports
- Strong attention to detail and problem-solving skills
- Computer proficiency, including Microsoft Office and the Internet
- Experience and/or ability to deal with multicultural clients
- Ability to maintain self‑control in stressful situations
- Individuals must have the ability to effectively communicate in verbal and written formats, collaborate with community service providers, and establish effective working relationships with clients
- Must have dependable transportation and current Texas Driver's License and active auto liability insurance
- Bilingual Spanish/English skills preferred, but not required
