- Education – Bachelor’s degree (BS) or Doctor of Pharmacy (Pharm
D) degree - Experience – Minimum 1 year of pharmacy experience
- Licenses and Certifications – Eligible for licensure by Texas State Board of Pharmacy (TSBP) upon hire and Texas license within 30 days; ACPE‑IV sterile compounding certification; BLS and ACLS certifications (AHA or ARC) upon hire, and ADV Cardiac Life Support within 180 days for inpatient positions
- Experience – 1 year of pharmacist experience
- Perform responsibilities in accordance with policies, procedures, and infection control standards, while complying with state and federal regulations.
- Accurate, thorough, and timely computer order entry; review medication profiles and be aware of patient charges.
- Review medication orders for therapeutic appropriateness, duplications, interactions, and overall safety; document interventions such as ADRs.
- Multi‑task and problem‑solve independently with minimal supervision; exercise judgment and lead when necessary.
- Enforce formulary system and deliberate non‑formulary acquisitions; participate in cost‑effective pharmaceutical decisions.
- Communicate drug information verbally and in writing to physicians and nurses; provide patient/family education on drug therapy and side effects.
- Manage technicians and supportive personnel; delegate tasks and validate accuracy of IV, central, and automated dispensing medications; audit controlled substances in accordance with guidelines.
- Engage in continuous education, orientation, and training; take on additional projects as needed.
- Support pharmacy committee initiatives and therapeutic monitoring of drug therapies.
- Perform additional duties as assigned.
