Job SummaryAs a Bereavement Coordinator, your primary mission is to provide coordinated care and counseling for hospice families across all age groups. You will assess bereavement needs, provide direct grief support, and facilitate referrals to community resources to ensure families receive the care they need during and after their hospice journey.
Key Responsibilities- Family Assessment: Assess hospice families to identify specific bereavement needs as evidenced by clinical documentation and community resource referrals.
- Counseling & Support: Provide continued bereavement support and grief counseling to hospice families, ensuring their emotional needs are met following a loss.
- Interdisciplinary
Collaboration:
Work closely with the Interdisciplinary Team (IDT) to promote the coordination of bereavement care and ensure a seamless transition for the family. - Community Advocacy: Maintain a working knowledge of public and private eligibility standards and community resources to provide effective referrals.
- Professional Excellence: Meet mandatory continuing education requirements and promote Agency philosophy and administrative policies to ensure the highest quality of care.
- Documentation: Maintain accurate and timely documentation of all assessments, communications, and support provided to families.
- Education: Master’s degree in Social Work or Counseling (MSW, LPC, or LCSW) is required.
- Licensure: Current professional license (LCSW, LMSW, or LPC) and a valid Driver’s License.
- Experience: Proven experience in bereavement and grief counseling.
- Skills: Strong interpersonal skills and a working knowledge of community resources.
- Requirements: Reliable transportation and valid auto liability insurance.
- Ability to make quick decisions and acquire resources under stressful conditions.
- Commitment to professional growth and maintaining clinical competency.
This position involves traveling locally to visit families in their homes and working in an office setting. You should be comfortable working in various conditions, including occasional exposure to infectious diseases and unpleasant weather. The role requires prolonged standing, walking, and sitting, as well as the ability to meet deadlines and family needs under stressful conditions.
