CoreResponsibilities
- Comprehensive Assessment
:
Identify psychosocial, financial, and environmental needs for hospice patients to guide the Plan of Care (POC). - Clinical Counseling
:
Provide direct counseling to patients and families, specifically focusing on advanced care planning and grief/loss. - Interdisciplinary Collaboration
:
Attend team meetings and collaborate with the Interdisciplinary Team to ensure seamless care coordination. - Community Advocacy
:
Maintain an expert working knowledge of community resources and public/private eligibility standards to assist patients with resource acquisition. - Clinical Documentation
:
Maintain accurate clinical records and team meeting minutes.
- Education
:
Master’s Degree in Social Work (MSW) from a state-accredited program. - Licensure
:
Current Texas Certification (LMSW). - Experience
:
At least one (1) year of social work experience in a health care agency. - Skills
:
Strong interpersonal skills and a deep understanding of community resources. - Compliance
:
Must pass criminal background checks and provide proof of TB testing and a Hepatitis profile/acceptance. - Transportation
: A valid Texas Driver’s License, reliable transportation, and valid auto liability insurance for local travel to patient homes.
This position involves traveling locally to visit patients in their homes. You should be comfortable working in various conditions, including occasional exposure to infectious diseases and unpleasant weather. The role requires prolonged standing, walking, and the ability to make quick decisions under stressful conditions to meet patient needs.
