Experience and EducationRequired
Education:
Undergraduate degree
Experience:
1 year to two (2) years in Speech Therapy
Licenses and
Certifications:
BASIC LIFE SUPPORT course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire
Valid Texas driver's license, proof of driver's insurance Upon Hire
Speech Therapist current licensure Upon Hire
Preferred Experience:
Home health experience.
Evaluates patient's home environment and communicative status of patient.
Develops plan of care (POC/POT) in coordination with physician (when required).
Provides patient care and coordinates the care with other disciplines and team members with the assistance of the Clinical Therapy Manager.
Coordinates patient care with other disciplines and team members.
Documents care and interventions.
Participates in the implementation of the Quality Improvement Plan.
Maintains a safe and healthy environment.
