Licensed Social Worker (MSW) – Home Health & Hospice (Part-Time)
Location:
Sunnyvale, TX. Pay: $35–$40 per visit (based on experience).

Position SummaryWe are seeking a dedicated and empathetic Licensed Social Worker to join our Home Health / Hospice team. The Social Worker provides psycho-social assessments, emotional support, care coordination, and community resource planning for patients and families facing acute, chronic, or end-of-life conditions. This role is an essential part of our interdisciplinary care team.
Responsibilities- Conduct comprehensive psycho-social assessments
- Develop and implement individualized care plans
- Provide emotional support and counseling to patients and families
- Assist with advance care planning and end-of-life decision support
- Connect patients and families to community resources and financial assistance programs
- Participate in interdisciplinary team meetings
- Maintain accurate and timely clinical documentation
- Support crisis intervention and bereavement services (hospice)
- Ensure compliance with federal, state, and agency regulations
- Master’s Degree in Social Work (MSW) from an accredited program
- Current state licensure
- Minimum [1 year] experience in healthcare, home health, or hospice preferred
- Strong communication and documentation skills
- Ability to work independently in a home-based setting
- Valid driver’s license and reliable transportation
- Experience in hospice or palliative care
- Knowledge of Medicare/Medicaid regulations
- Bilingual skills (if applicable)
- Competitive visit pay ($35–$40) (based on experience)
