Licensed Social Worker (MSW) – Home Health & Hospice (Part-Time)

Location:

Sunnyvale, TX. Pay: $35–$40 per visit (based on experience).

Schedule:

Part-Time | Flexible Scheduling | PRN

Position Summary

We are seeking a dedicated and empathetic Licensed Social Worker to join our Home Health / Hospice team. The Social Worker provides psycho-social assessments, emotional support, care coordination, and community resource planning for patients and families facing acute, chronic, or end-of-life conditions. This role is an essential part of our interdisciplinary care team.

Responsibilities
  • Conduct comprehensive psycho-social assessments
  • Develop and implement individualized care plans
  • Provide emotional support and counseling to patients and families
  • Assist with advance care planning and end-of-life decision support
  • Connect patients and families to community resources and financial assistance programs
  • Participate in interdisciplinary team meetings
  • Maintain accurate and timely clinical documentation
  • Support crisis intervention and bereavement services (hospice)
  • Ensure compliance with federal, state, and agency regulations
Qualifications
  • Master’s Degree in Social Work (MSW) from an accredited program
  • Current state licensure
  • Minimum [1 year] experience in healthcare, home health, or hospice preferred
  • Strong communication and documentation skills
  • Ability to work independently in a home-based setting
  • Valid driver’s license and reliable transportation
Preferred Qualifications
  • Experience in hospice or palliative care
  • Knowledge of Medicare/Medicaid regulations
  • Bilingual skills (if applicable)
