Medical Records Coder II

Methodist Health System seeks a detail-oriented Medical Records Coder II to accurately code inpatient and outpatient encounters in our Healthcare & Medical Services department. You will assign ICD-10-CM, CPT, and HCPCS codes, ensure DRG/APC accuracy, and support optimal reimbursement while maintaining strict compliance with payer, state, and federal regulations. Responsibilities include reviewing clinical documentation, querying providers for clarification, meeting productivity and quality benchmarks, and contributing to process improvements.

Responsibilities

• Review inpatient and outpatient medical records for completeness and accuracy
• Assign appropriate ICD-10-CM, CPT, and HCPCS codes according to guidelines
• Validate DRG/APC assignments and ensure accurate reimbursement
• Query providers to clarify ambiguous or incomplete documentation
• Ensure compliance with payer, state, and federal coding and billing regulations
• Meet established productivity and coding quality standards
• Collaborate with clinical and billing teams to resolve coding discrepancies
• Participate in coding audits and implement corrective actions as needed
• Maintain up-to-date knowledge of coding guidelines and regulatory changes
• Support process improvement initiatives within Health Information Management
