Medical Records Coder

Methodist Health System is seeking a detail-oriented Medical Records Coder 2 to accurately code inpatient and outpatient records within our Healthcare & Medical Services department. This role reviews clinical documentation, assigns ICD-10-CM and CPT/HCPCS codes, and ensures compliant DRG/APC assignment to support appropriate reimbursement and quality reporting. The Medical Records Coder 2 works within our EHR, collaborates with providers for clarification, and upholds HIPAA standards.

Responsibilities

• Review inpatient and outpatient medical records for completeness and accuracy
• Assign ICD-10-CM and CPT/HCPCS codes in compliance with coding guidelines
• Ensure accurate DRG/APC assignment to support appropriate reimbursement
• Work within EHR systems to verify documentation supports code selection
• Collaborate with clinicians to clarify diagnoses and procedures
• Monitor and maintain coding quality and productivity standards
• Protect patient confidentiality and comply with HIPAA regulations
• Assist with audits, denials, and documentation improvement initiatives
