Medical Assistant - Chronic Care Management

We are seeking a dedicated and experienced Medical Assistant with expertise in Chronic Care Management (CCM) to join our healthcare team in Dallas, TX. The ideal candidate will have hands‑on experience supporting patients with chronic conditions, assisting providers in delivering comprehensive care, and ensuring effective management of chronic illnesses.

• Assist healthcare providers in managing patients with chronic conditions such as diabetes, hypertension, COPD, and others.
• Perform patient intake, including vital signs, medical history updates, and medication reconciliation.
• Coordinate and document Chronic Care Management services in compliance with CCM guidelines.
• Educate patients on disease management, medication adherence, and lifestyle modifications.
• Schedule follow-up appointments and facilitate communication between patients and providers.
• Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
• Support care coordination efforts to improve patient outcomes and reduce hospital readmissions.

• Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) preferred.
• Proven experience in Chronic Care Management (CCM) within a clinical setting.
• Strong knowledge of chronic disease processes and management protocols.
• Excellent communication and patient education skills.
• Familiarity with electronic health records (EHR) and documentation standards.
• Ability to work collaboratively in a fast‑paced healthcare environment.
• High level of professionalism and attention to detail.
