Role Snapshot

• RN Case Manager – Home Health
• $40/hr
• Full-time (36–40 hrs/week)
• Onsite
  • In-home visits across the DFW region
• One patient at a time. Clear care plans. Predictable hours. New grads welcome

• Provide skilled nursing care in patients’ homes
• Follow clear care plans and clinical protocols
• Monitor vitals, medications, and medical equipment
• Document visits in Kantime EMR (modern system)
• Coordinate care with families and clinical team
• Support safe transitions and continuity of care

• Active Texas RN license
• Available for 36–40 hrs/week
• Able to complete home visits in assigned counties
• Comfortable working in home health settings

• Kantime EMR or EVV experience
• Prior home health experience
• Bilingual (Spanish)

• Pay: $40/hr (paid training included)
• Health, dental, vision insurance
• PTO + 401(k)
• Paid onboarding: 1-week lab + 3–7 days precepting
• Clear paths into leadership roles

• Full-time: 36–40 hrs/week
• 8–12 hour shifts available
• Travel required for in-home visits
• 1-week onboarding in Dallas (Mockingbird Ln)
